That's usually caused by SELinux or AppArmor, depending on what 
distribution you use.
On the command line, as root do
     clamdscan /var/zendto
and see what it says. If it spits permissions errors, that's usually 
SELinux/AppArmor getting in the way, you can confirm this by trying to 
clamdscan /root (when you're logged in as root).

If it's not that, then it's permissions problems on /var/zendto/incoming.

Have you installed php-pecl-apc? You need to install the apc package, as 
it's not usually included in systems by default.

Correct, you get one or the other. If you want both, then just run 2 
copies of ZendTo on different ports or on different URLs (i.e. make 2 
virtual hosts in Apache, and have 2 copies of ZendTo, one in /opt/zendto 
and one in /opt/myzendto).
